What causes a double chin, according to Korean surgeons?
Four things, alone or mixed: fat, loose neck muscle, loose skin, and a short or set-back chin. Dr. Yoo Han-soo of Riting Plastic Surgery in Busan laid out that list in a 2024 interview and said the causes differ person to person.
When is liposuction enough?
When the fullness is mostly fat and your skin can still shrink back over the new shape. That's usually younger patients, or a double chin that came with weight gain rather than age.
Korean clinics usually send you home in a chin strap (압박밴드). How long you wear it varies by surgeon, so get it in writing before you book flights.
Lipo removes fat. It doesn't lift skin. Taking fat out from under loose skin can leave the neck looking emptier and looser than before.
What is 근육묶기, and when is it added?
It's platysma tightening: the surgeon stitches the thin, wide muscle at the front of the neck so it holds the area under the chin tighter. Korean clinics offer it with lipo as one package, and Dr. Yoo said it makes a visible difference when the problem is muscle sag rather than fat.
He tied it to aging: the muscle loosens and the front of the neck sags with it.
When do you need a lower face or neck lift instead?
When the skin itself has dropped. Dr. Kim Sun-cheol, writing in a Korean outlet in February 2026, said that once sagging reaches the neck, the plan should lift the whole lower face from jaw to neck. He also warned that neck skin is thin, and pulling it too hard looks unnatural.
