How Do Korean Surgeons Correct Uneven Breasts?

Different implant sizes on each side, a one-sided lift, fat grafting or nipple repositioning. The thresholds one Seoul clinic uses (50 cc, 1 cm), what a Korean 3D-scan study found, and what implants can't fix.

Short answer: Mostly with a different implant on each side, sometimes plus a lift on the lower side, fat grafting, or moving one nipple. One Gangnam clinic starts treating it as a correction case at a 50 cc volume gap or a 1 cm difference in nipple or fold height. Differences that come from the rib cage or spine can be reduced, but implants won't erase them.

Korean patients call it 짝가슴, roughly "odd pair." Almost everyone has some. The question for surgery is whether it's big enough to plan around, and what's causing it.

How common is breast asymmetry?

Close to universal. UNE Plastic Surgery's asymmetry guide puts it bluntly: breasts that are exactly alike are rare. It cites a 1980 mammogram study of 174 healthy women in which 55% had a larger left breast.

Korean data says the same. In a 2023 study of 87 Korean women scanned in 3D before augmentation, the left breast measured about 9% larger than the right on average, and the nipples sat at different distances from the midline (9.3 cm versus 8.5 cm). Both gaps were statistically significant.

When does asymmetry need its own plan?

When it's visible. UNE uses these thresholds, and says any one of them can show:

MeasurementTreated as a correction case at
Breast volume difference50 cc or more
Nipple height difference1 cm or more
Inframammary fold height difference1 cm or more
Nipple-to-midline difference5 mm or more

Expect the surgeon to look at you both standing and lying down, and to take photos and measurements before suggesting anything.

One more thing UNE flags: if the difference appeared or grew recently, get imaging before cosmetic surgery. A new change in one breast is a medical question first.

What are the ways to correct it?

The method follows the cause.

  • Different implant sizes. The usual answer when volume is the main difference.
  • A lift on one side. For one breast sitting lower or sagging more. See breast lift with implants.
  • Fat grafting. For contour gaps an implant can't fill. More in fat transfer breast augmentation.
  • Moving the nipple and areola. When nipple position is the main difference.

Some plans touch only one breast. Others adjust both so they meet in the middle. Ask which one the surgeon proposes and why.

What can't implants fix?

The skeleton. If the rib cage itself is uneven (흉곽 비대칭) or the spine is curved, UNE says some difference will remain after surgery.

Two conditions come up often in Korean clinic writing:

  • Pectus excavatum (오목가슴), a sunken breastbone. In a series of 11 women UNE cites, 4 had a smaller right breast. Surgeons chose a different size per side with sizers and sometimes added fat to the remaining hollow.
  • Tuberous breasts (뾰족가슴), a narrow, pointed breast with a high fold, often uneven. UNE says an implant alone can make it look more pointed. The base usually needs releasing as well.

If either describes you, say so before the consultation. It changes the plan.

How do you know if it worked?

By setting the target before surgery. UNE's guide says plainly that left and right rarely end up identical, and satisfaction depends on agreeing in the consultation what's realistic. UNE also recommends standardized photos from five angles before surgery, so the result can be compared fairly.

Correction can be done as part of a revision too, though that's more complex. See breast implant revision.

How we help

  • Free, itemized quotes from 2 to 3 specialist clinics, with the cc on each side and any lift or fat graft listed separately.
  • An interpreter in the consultation room while you're measured, so you understand which difference the surgeon is correcting.
  • Scheduling across hospitals, so you can compare two surgeons' plans in one trip.

Frequently asked questions

Can breast augmentation fix uneven breasts?

Often, mostly by using a different implant size on each side. A lift, fat grafting or nipple repositioning may be added depending on the cause.

How much difference counts as breast asymmetry?

One Gangnam clinic plans a correction at a 50 cc volume difference, a 1 cm difference in nipple or fold height, or 5 mm in nipple-to-midline distance.

Can you get a different implant size on each side?

Yes. It's the usual way Korean surgeons correct a volume difference.

Will my breasts be perfectly even after surgery?

Rarely. Differences from the rib cage or spine can be reduced but not removed, so agree on a realistic target at the consultation.
