Can You Get En Bloc Breast Implant Removal in Korea?
Yes. Korean clinics remove implants alone, with part of the capsule, or with the whole capsule, and en bloc is one form of the last. When a Seoul surgeon will say the capsule should come out, when they'll call it overtreatment, and what removal costs on Korean apps in 2026.
Short answer: Yes. Korean plastic surgeons offer three levels: implant only, implant plus part of the capsule, or total capsulectomy (전피막절제술), with en bloc meaning the implant and capsule come out together in one sealed piece. Korean app listings showed removal alone at about ₩1–3.5 million ($700–2,500) in 2026. Seoul surgeons generally reserve total removal for a diseased capsule, and some openly call it overtreatment when the capsule is healthy.
If you're asking about en bloc, you've probably met the term on US explant forums. The Korean conversation is a little different, starting with the vocabulary.
What's the difference between en bloc and total capsulectomy?
Both remove the whole capsule, the scar tissue shell your body built around the implant. En bloc takes the implant and capsule out as one closed unit, without opening the capsule. A total capsulectomy can take the capsule out in pieces or after the implant is removed.
Korean clinic pages mostly say 전피막절제 (total capsule removal) and rarely use "en bloc." Ask which one the surgeon means.
When does a Korean surgeon say the capsule must come out?
When it's diseased. Seol Cheol-hwan of JW Jeongwon Plastic Surgery, interviewed in September 2026, listed four situations:
- Severe capsular contracture
- Heavy calcification of the capsule
- Large seromas (fluid collections) that keep coming back
- Severe inflammation or granulation tissue
For a simple size change, position fix, or even removal after a rupture, he said a thin, healthy capsule doesn't need to come out. Removing a healthy capsule in every case was, in his word, overtreatment (과잉수술).
The reason is risk: total removal is long and hard, with bleeding, thinner tissue, muscle damage, numbness and, rarely, a collapsed lung.
What if BIA-ALCL is suspected?
Then complete removal is the treatment, not a preference. Korea's device regulator MFDS (식약처) tells hospitals to confirm it first by draining the fluid under ultrasound for lab testing, and to hold surgery until then so tumor cells aren't spread. For disease that hasn't spread, removing the implant and entire capsule is described as curative.
Korea confirmed its first case in 2019, in a patient with an Allergan textured implant. A 2021 review in the Korean Medical Association's journal counted three Korean cases and no deaths. MFDS doesn't recommend removing implants in women without symptoms.
Which incision is used?
Often the fold under the breast, which gives the surgeon a direct view of the whole capsule. Seol says endoscopic total removal through the armpit is now possible too, and the choice depends on your earlier scars and the capsule's condition. See armpit or under-breast incision.
How much does implant removal cost in Korea?
Less than a new augmentation. UNE Plastic Surgery's 2026 survey of Korean app listings found removal alone at about ₩1–3.5 million across seven listings. The survey didn't price en bloc separately, so capsule work needs a quote after an exam. The median revision in the same survey was ₩8.8 million ($6,300); see revision process and cost.
What will your breasts look like afterward?
It depends on how long the implant was in, its size, your skin and whether you've had children. Some women look close to how they did before. Others are left looser and lower, which is why Korean surgeons often discuss a lift or fat grafting in the same operation. See breast lift with implants and fat transfer.
What should you ask before booking?
- Implant only, partial capsule, total capsulectomy, or en bloc, and why for my capsule?
- Will the capsule be sent to pathology, and can I have the report in English?
- Which incision, and will there be drains?
- What does the capsule look like on ultrasound today?
How we help
- Free, itemized quotes from 2–3 specialist clinics, with the capsule plan, pathology and any lift or fat graft on separate lines.
- An interpreter in the consultation room and at the ultrasound, so the en bloc question gets a precise answer.
- Scheduling across the clinic and any outside imaging, plus airport pickup. No charge to you.

Frequently asked questions
Do Korean surgeons perform en bloc capsulectomy?
Yes. Korean clinics more often describe it as total capsule removal (전피막절제), so ask whether the implant and capsule will come out as one sealed piece.
Is en bloc removal always necessary?
Seoul surgeons generally reserve total capsule removal for severe contracture, calcification, recurring seromas, inflammation or suspected BIA-ALCL. Some call removing a healthy capsule overtreatment.
How much does breast implant removal cost in Korea?
Korean app listings surveyed in 2026 showed removal alone at about ₩1–3.5 million, roughly $700–2,500. Capsule removal costs more and needs an exam to quote.
Should I remove textured implants if I have no symptoms?
MFDS, Korea's device regulator, doesn't recommend removal in women without symptoms.
