Is It Normal for One Side to Look Different After a Facelift?
When uneven swelling after a facelift is part of healing, when it's a same-day call to the clinic, how long Korean surgeons say to wait before judging the result, and why they put revision at six months to a year.
Short answer: In the first few weeks, usually yes. Swelling rarely settles evenly, and Gangnam Unni's guide says the wide, swollen look usually goes down within a month. One side that suddenly swells much more, with pain that's getting worse, is different: call the clinic that day. Korean surgeons wait six months to a year before calling a facelift asymmetric and planning a correction.
Day 6, the mirror, one cheek fuller than the other. It's one of the most common worries in the first two weeks.
Most of the time it sorts itself out. This post covers how to tell which kind you have.
Why does one side swell more than the other?
Because the two sides were never identical, and they aren't operated on identically. Most faces start out uneven, and the surgeon may lift one side further to match them.
A Korean patient in our choosing a surgeon post noticed uneven swelling and scarring on the side that needed more lifting.
Before surgery, ask the surgeon to point out your existing asymmetry in the photos. It makes week two much less alarming.
When is uneven swelling an emergency?
When one side swells quickly and keeps getting tighter, harder or more painful, especially in the first day or two. That can be a hematoma, blood collecting under the skin. Asan Medical Center says the drain placed after a facelift is there to prevent one.
Don't wait for the next scheduled visit. Call that day. Our post on what raises hematoma risk lists the warning signs.
What about a smile that looks uneven?
That's usually the nerve, not the swelling. Gangnam Unni's side-effect guide says most expression problems after a facelift are temporary, caused by swelling pressing on the facial nerve, and cites an analysis of about 15,000 patients in which permanent motor nerve damage was under 0.05%.
A December 2025 Kormedi piece said most cases recover within months. If it's lasting or getting worse rather than better, tell your surgeon. The same Gangnam Unni guide gives that advice for any symptom.
When can you judge the final result?
Not before three months. Gangnam Unni's facelift guide puts full settling at three to six months.
Some changes show up late. The same guide says a pulled-down earlobe (칼귀, "knife ear") often appears six months to a year after surgery. See pixie ear after a facelift.
When do Korean surgeons do a revision for asymmetry?
Usually at six months to a year. Dr. Kwon Young-hoon of Rimo Plastic Surgery wrote in 2024 that facelift revision is generally advised from six months to a year after the first operation, because tissue is likely to be stuck together (유착) from the first surgery.
The Kormedi piece added two cautions: scar tissue makes revision harder and raises the risk to nerves and blood supply, and a second lift pulled too hard can look tight and unnatural. It recommended finding the exact cause and correcting the smallest area possible.
Our revision guide covers what the process looks like.
What should you do from home?
Take photos the same way every week: same light, same distance, face straight on, mouth relaxed, then smiling. Send them to the clinic with the date.
That record is what a surgeon needs at the six-month mark. It's also what a second surgeon will ask for. More in facelift aftercare after you fly home.
